The OHS Athletic Department requires that all athletes going out for a sport submit the following forms:
- Mandatory Physical/Permission Form
- Parent and Athlete Pledge Form
- 2009 Athletic Booster Member Application
Parents may want to join a mailing list to receive up–to–date information on teams, schedule changes, etc. Just email Melissa Knight to be added to the list.
