This form is to be used when any agreement is being established between your office/school/program and an individual for any hourly assignment, extended day assignment including homebound services and afterschool activities, or short term activities such as one or multi-day workshop presenters. 

Note:  The Budget Director’s signature (individual responsible for administering the specific budget) must be included prior to submission for payment.
